How many of you keep a log? What bbok or sheet do you use?

How many of you guys keep a detailed and accurate logbook with blood sugars, boluses, exercise, etc.
   Be honest.  If so, what book or sheet do you use?  What do you bring to your endo or does he/she
   download your meter and/or pump info.

Here's a sheet I found which I like.  I also posted a PDF version on my Profile which you candownload and print.

 

We just use the one that the dr gives us.  I don't have her pump yet so we have not gotten into detailed record keeping.  I copy her BG numbers daily and we turn 3 months in every 3 months of course lol when we see her dr.  We are in the process of getting her pump right now.

I log my numbers for the 2 or 3 weeks previous to my endo appointments. If CareLink worked with Vista 64-bit system I would use that... but they haven't got a driver for it yet. Come to think about it... I'm going to email them and check up on this.

I had been using the Minimed log books, which are great.  They're the only paper log book I've ever seen that actually had enough room for many BG tests, notes about food, exercise, illness etc and boxes to notate things like infusion set changes, ketones etc.  Last year I got lazy filling them out because I was always leaving it on my desk at work or on the table at home.  I'd never remember to catch up the next day.  Now I use a super basic spreadsheet on Google Docs, a suggestion I found on another site.  I just keep it open at work or at home and I can access it anywhere with an internet connection.  You can put in as much or as little info as you need.  You can also send a link to anyone else to share your documents.

I also download my pump to Carelink and enter all of my BGs in the pump so that gets downloaded there as well.

 

I really don't use one anymore - I stick to downloading everything from my pump and meter to my PC and look at everything there.  With the event section of my meter, I can usually know what's been going on - exercise, hypo etc.   I find that if I keep a detailed meal and exercise (or lack there of) log that I'm able to know why I went high or low.  I don't do it all the time but I see my doc 4 times a year so every 3rd month I write down everything I eat - even the one off cracker or something.  It seems to work well.  When I did keep a log, I had a personal one I used that I printed out on a spreadsheet so I could either put it in a binder or just carry on sheet with me.  It looked like this:   (the section below comments was for what I ate)

Hey Paul -  Yes - I created it - just combined stuff that I liked from various logs.  I have it in both a Word doc and in Excel if you want a copy.  I like the word one (the one I posted)- I add a header to it with my name and doc's name as well.  I'll try to figure out how to post it on my profile when I get home from work.

Yeah - I haven't used a log book since the Ice Age.  The only thing I do now that I'm on the pump is download the info from it for my endo - about 2 weeks worth. I guess having had it so long I just don't bother - and your A1C's will reflect how your doing as well. It is good to use a log book tho' if you are new to the diabetes roller coaster ride - and even if you aren't  - some people just need to keep a log book - as it's in their nature to do so.  The only time I write something down is when things are going wacky - and I have an agenda book I use for work - and just scribble notes in there.

Could you send it to me aswell Doug?? I have such issues with logging. GAHH I need to start because I sometimes wonder what my BG is doing and try to use my One Touch Ultra Smart logging tools as often as I remember but don't always.. only when I lower my basal (Lantus) etc.
