if you use Minimed quick set infusion sets, and have a box with a lot number that starts with an 8, Minimed has recalled the product for infusion set failures.
UPS supposedly has a list of quick set customers, and will be delivering a free box of quick sets as a replacement, in the next "couple of days" - along with a better explanation of the failures, and info on how to replace your inventory if you have more boxes of "Lot 8" sets.
When I sopke to Minimed they said that failures were over 2%, which triggered a product recall.
