Help with dissertation study of young adults with TID

Are you an adult with type 1 diabetes? We want to hear from you! We are conducting an anonymous, online survey about personal relationships and health management in young adults. You are eligible to participate if you (a) have been diagnosed with type 1 diabetes at least one year ago by a medical professional, (b) are between the ages of 18 and 35, (c) have been in a dating, cohabitating, or marital relationship for at least six months, (d) live in the United States, and (e) live independently of your parents or guardians. We would also appreciate it if you would help us share the invitation to participate in this study with others you think might be interested in participating.

The survey should take approximately 20 minutes to complete, and you can access it here: http://bit.ly/1XL1Cuy. In the survey, you will be asked to report your most recent HbA1c level, so please have that available if possible. After you complete the survey, you can choose to enter your contact information for a 1 in 40 chance to win one of five $10 Amazon gift cards.
